Work Environment

This role is set in a home care environment, where our therapists work one-on-one with patients in their homes. Key Responsibilities

• Adhere to state practice acts and regulations governing physical therapy practice to ensure compliance and high standards of care.

• Conduct thorough examinations and evaluations of patients, assessing their functional limitations and developing tailored treatment plans based on best practices and individual needs.

• Implement and modify therapy protocols according to the latest clinical evidence, patient progress, and any evolving health care concerns.

• Document all aspects of patient care meticulously, including initial evaluations, treatment plans, daily notes, progress reports, and discharge summaries, ensuring accuracy and adherence to regulatory requirements.

• Engage in proactive consultations with interdisciplinary healthcare teams, recognizing when a patient's condition necessitates additional medical or rehabilitation consultations.

• Plan and execute effective discharge strategies and follow-up procedures to optimize patient recovery trajectories and provide guidance for continued care.

• Participate in facility meetings and relevant training sessions to remain updated on advancements in the field and ensure compliance with all corporate and clinical service requirements.

• Assist in removing barriers to access when necessary, and actively take part in initiatives aimed at improving service delivery and patient satisfaction.

Patient Population / Client Focus

Our patients primarily consist of individuals recovering from orthopedic surgeries or dealing with ongoing conditions that affect their movement and functionality. As a home care physical therapist, you will play a vital role in supporting patients of varying ages and backgrounds, each with their unique rehabilitation needs. You'll develop a strong rapport with your patients, tailoring therapies to enhance their quality of life and self-sufficiency within their home environments.

Qualifications

• Doctorate of Physical Therapy (DPT) from an accredited program.

• A valid state license as a Physical Therapist (PT) in Florida, issued by the Physical Therapy Examining Committee.

• Minimum of two (2) years of experience in outpatient orthopedic settings, showcasing a solid foundation in physical therapy practices.

• Current CPR Certification, ensuring readiness to respond to emergencies.

Preferred Experience

• Experience in home health care settings is a plus, providing insight into the unique challenges and advantages of delivering therapy outside a traditional clinical environment.

• Familiarity with electronic health record (EHR) systems to streamline documentation and patient management.

Compensation and Benefits
