Key Responsibilities\n

As a Senior LCSW at CHDC, your primary responsibilities include, but are not limited to:

• Delivering comprehensive clinical behavioral health assessments and interventions, ensuring alignment with the health center's mission and quality standards.

• Facilitating access for mental health consumers to both physical and behavioral healthcare services, enhancing integrated care experiences.

• Providing patient education on available resources and services, cultivating personal responsibility in disease prevention and health promotion.

• Collaborating with an interdisciplinary treatment team, actively participating in meetings to discuss patient care plans and coordinate services.

• Documenting patient interactions and clinical assessments accurately and thoroughly, ensuring compliance with state and federal regulations.

• Reporting crises and unusual incidents promptly to the Medical Director and Chief Medical Officer, guaranteeing swift and appropriate intervention.

• Implementing evidence-based screening tools to assess healthcare needs and health statuses, using collected data to inform treatment strategies.

• Engaging in team-oriented efforts to support patients with Opioid Use Disorder in collaboration with our Center of Excellence team.

• Actively participating in quality improvement initiatives, offering insights based on patient interactions and clinical findings.

This role focuses primarily on patients receiving hospice care, including individuals facing terminal illnesses who require support for emotional, psychological, and social challenges. The ideal candidate will possess the sensitivity and expertise to provide comfort and care to patients in their final stages of life, along with their families. Qualifications\n

The successful candidate will meet the following qualifications:
