Key Responsibilities\n

• Conduct comprehensive biopsychosocial assessments for hospice patients, focusing on mental health, substance abuse, and medical needs, ensuring culturally competent and age-appropriate care.

• Offer individual and group counseling services, effective treatment planning, crisis intervention, and support for patients and their families.

• Skillfully document mental health evaluations, progress notes, and suicide risk assessments in accordance with HIPAA regulations and internal policies.

• Collaborate with healthcare providers to escalate patient needs and advocate for necessary services, ensuring holistic patient care is prioritized.

• Act as a liaison to community resources, facilitating referrals to local agencies for additional support, including financial assistance and counseling services.

• Engage in quality improvement initiatives, contributing to the development and implementation of evidence-based practices that enhance service delivery.

Patient Population / Client Focus\n

This position focuses on providing support to hospice patients and their families during one of the most critical times of their lives. You will work with a diverse client population, including individuals from various cultural, economic, and social backgrounds, addressing their unique emotional, psychological, and practical needs through empathetic and knowledgeable care.

Qualifications\n

• A valid and active Licensed Clinical Social Worker (LCSW) license in the state of Oregon is required.

• A Master’s degree in Social Work is mandatory, alongside a minimum of three (3) years of post-licensure clinical experience in mental health settings, particularly in hospice, palliative care, or similar environments.

• Proficiency in counseling techniques, crisis intervention skills, and the ability to exercise independent judgment in providing direct service are essential.

• Strong interpersonal and communication skills, with a proven ability to work effectively with interdisciplinary teams and diverse groups of clients.

Compensation and Benefits\n
