Key Responsibilities\n

• Conduct comprehensive assessments to identify patient needs, develop individualized care plans, and establish measurable therapeutic goals in partnership with clients.

• Regularly evaluate and adjust treatment plans in collaboration with patients and their caregivers, leveraging insights from ongoing progress reviews.

• Facilitate individual and group psychotherapy sessions, utilizing established therapeutic interventions (e.g., cognitive-behavioral therapy, crisis counseling) tailored to each client's unique circumstances.

• Work in conjunction with healthcare providers to implement and uphold treatment practices that align with the organization’s policies, Mental Health, and Substance Abuse Standards.

• Assess family dynamics and functioning within various life contexts—such as work, family structures, and daily routines—to determine necessary support systems and treatment pathways.

• Maintain clear, concise documentation related to patient progress and program adherence, ensuring compliance with industry regulations and standards.

In your role as a Licensed Clinical Social Worker at Path to Wellness, you will primarily serve individuals and families navigating the complexities of behavioral health issues, including those diagnosed with mental disorders and substance use challenges. Compensation and Benefits\n
