Key Responsibilities\n

• Evaluate and assess the needs of patients and their caregivers on a continuous basis, modifying care plans in collaboration with them as necessary.

• Establish and develop tailored plans of care based on comprehensive assessments and set specific, measurable goals focused on desired outcomes.

• Implement individualized care plans while ensuring alignment with the established protocols of the organization, as well as Mental Health and Substance Abuse Standards.

• Provide therapeutic interventions, including crisis counseling, for individuals, families, and groups, catering to their unique situations and needs.

• Facilitate engaging individual psychotherapy sessions and dynamic group therapy settings that foster community and support.

• Perform a thorough assessment of family systems to evaluate the functioning of clients within their daily living environments, identifying additional support, resources, and treatment when necessary.

• Regularly monitor and evaluate client progress toward goals, adjusting care plans to ensure alignment with therapeutic objectives.

• Demonstrate sensitivity and awareness toward the nuanced needs of patients and families, ensuring personalized care at all times.

Qualifications\n

Compensation and Benefits\n
