Key Responsibilities\n

• Conduct thorough assessments and ongoing evaluations to identify patient needs and develop effective treatment plans.

• Communicate regularly with supervisors and interdisciplinary team members regarding client progress, fostering an integrated approach to care.

• Organize and manage a flexible appointment schedule, including virtual sessions and necessary home visits, to ensure continuity of care.
\n
• Coordinate and facilitate home visits for in-person assessments, chart reviews, and staff interviews as needed.
\n
• Ensure that all therapeutic interventions align with established program plans and are executed effectively.
\n
• Document progress through detailed monthly and quarterly notes, maintaining up-to-date client records for collaborative care.

• Actively participate in interdisciplinary team meetings, sharing insights and facilitating discussions on client progress and treatment efficacy.

• Provide training and resources to colleagues to ensure they are equipped to implement program protocols and documentation standards.
\n
• Engage in follow-up consultations to continue providing support and adjustment of treatment plans as necessary.

• Serve as a liaison between the public, mental health professionals, and community resources to foster supportive networks for patients.

• Help design and improve training materials aimed at enhancing staff development and program deliverables.

As a Licensed Clinical Professional Counselor at Madina Care Consulting, you will work with a diverse clientele, including children, adolescents, and adults facing a variety of mental health challenges. Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a related mental health field is required.
