Key Responsibilities\n

• Conduct ongoing assessments through telehealth consultations to evaluate client needs and develop personalized care plans.

• Maintain regular communication with interdisciplinary team members, clients’ families, and other stakeholders to monitor patient progress and adjust treatment as needed.

• Manage a tentative schedule of virtual appointments, ensuring timely follow-ups and continuity of care.

• Facilitate virtual home visits and observations as required to gain comprehensive insight into a client’s situation.

• Implement treatment plans effectively and consistently, ensuring adherence to ethical and legal standards in all interactions.

• Participate actively in team meetings to discuss patient care strategies and improvement initiatives.

• Document monthly and quarterly progress notes that reflect patient advancements and clinical decisions made during sessions.

• Organize training sessions for office staff and team members to promote competence in program implementation and documentation practices.

• Conduct follow-up interviews and virtual visits as a means of providing ongoing support to clients.

• Maintain accurate records that can be easily accessed by relevant parties to ensure transparency and informed decision-making.

• Act as a liaison with the community and external mental health organizations, aiding in resource development and client advocacy.

• Contribute to the development of training materials that enhance organizational programs and client services.

Patient Population / Client Focus\n

As a Licensed Professional Counselor at Mental Thrive, you will work with a diverse clientele, including adolescents and adults facing a range of mental health challenges. This may include anxiety, depression, trauma, substance misuse, and relationship issues. Your expertise will play a critical role in improving their mental health outcomes and overall well-being.

Qualifications\n

• A Master's or Doctorate Degree in Counseling, Psychology, or a related field is required.

• Active and unrestricted licensure as a Professional Counselor (LPC) in the state of New Jersey is mandatory.

• At least 2 years of counseling experience in a clinical setting is required.

• Experience in family therapy, couples therapy, trauma-informed care, or substance misuse is highly desirable.

• In-depth knowledge of legal and ethical standards governing counseling practices.
