Key Responsibilities\n

• Prepare and maintain accurate and timely documentation in compliance with state regulations and organizational standards.

• Utilize a variety of evidence-based screening tools as determined by the clinical team to effectively assess client needs.

• Collaborate with interdisciplinary teams to create and implement positive behavior support plans that are aligned with individual client goals.

• Serve as an essential liaison among service systems, state agencies, and community partners to enhance service delivery and coordination.

• Participate in licensing and accreditation processes, contributing to the overall standards of care within our facility.
