What You'll Do:

• Perform patient evaluations through history-taking, systems review, and specific tests and measures.
• Develop individualized treatment plans with patient-centered goals to reduce pain and restore function.
• Conduct ongoing reassessment of patient progress and modify plans as needed.
• Initiate and manage discharge planning from the first visit throughout treatment.
• Supervise and delegate to ancillary staff and students per state laws.
• Complete accurate, legible documentation within the established timeframes.
• Communicate effectively with patients, families, and healthcare team members to ensure optimal care.
• Educate patients and families on treatment plans and health management.
• Participate in quality improvement activities and uphold safety standards.

What You Need:

• Current state license as a Physical Therapist
• Experience in long-term care or relevant setting preferred
• Strong communication and patient assessment skills
• Ability to work as part of an interdisciplinary team
