Key Responsibilities\n

• Conduct comprehensive assessments to identify the individual needs of clients, tailoring treatment plans accordingly.

• Facilitate effective communication among supervisors, direct care teams, families, and other stakeholders to provide updates on client progress.

• Create and maintain a structured schedule for client appointments, home visits, and case observations to ensure consistent support and monitoring.

• Engage in home visits and in-depth chart reviews to gather data and insights that inform treatment strategies.
\n
• Implement and evaluate program plans, ensuring they meet regulatory standards and effectively address client needs.

• Participate actively in Interdisciplinary Team Meetings, contributing to discussions integral to comprehensive patient care.

• Document monthly and quarterly progress notes to track client development and adjust care plans as necessary.
\n
• Organize training sessions for staff, ensuring competency in program delivery and documentation processes.
\n
• Provide ongoing support through follow-up interviews and home visits to reinforce client progress and report on continued needs.

• Serve as a liaison with the larger mental health community to aid in developmental goals and resource sharing.

• Assist in creating training materials aimed at optimizing program frameworks for better patient outcomes.

Qualifications\n

• A Master’s or Doctorate degree in Counseling, Psychology, or a closely related field is required.

• Active and unrestricted licensure as a Professional Counselor in the state of Texas.

• Specialization in family therapy, couples therapy, trauma-informed care, or substance misuse treatment is highly desirable.

Preferred Experience\n
