Key Responsibilities\n

• Conduct comprehensive psychosocial assessments to evaluate patients' mental health needs, strengths, and overall well-being.

• Implement evidence-based therapeutic interventions, including Cognitive Behavioral Therapy (CBT) and Dialectical Behavior Therapy (DBT), tailored to the unique needs of hospice patients.

• Collaborate with an interdisciplinary team to create and maintain coordinated care plans that prioritize patient-centered goals and preferences.

• Educate patients and their families on diagnoses, treatment options, and medication management to facilitate informed decision-making.

• Provide individual, group, and family therapy sessions, utilizing a variety of modalities to address complex emotional and psychological needs.

• Assist patients in navigating the often-complex mental health system, advocating for their rights and ensuring access to essential therapeutic resources.

• Facilitate therapeutic groups, psychoeducation sessions, and discharge planning meetings while maintaining sound documentation using electronic health record systems.

Compensation and Benefits\n
