Key Responsibilities\n

As a Licensed Clinical Social Worker at Serving Children and Reaching Families, your day-to-day responsibilities will include:

• Conducting thorough biopsychosocial assessments to identify mental health, substance abuse, and medical needs for diverse age groups and cultural backgrounds within a primary care setting.

• Providing a range of behavioral health services, including individual and group counseling, crisis intervention, intake assessments, treatment planning, and ongoing evaluation of client progress.

• Collaborating with multidisciplinary teams to escalate complex patient needs, ensuring that referrals to additional healthcare providers or community resources are handled proactively.

• Engaging in data collection and quality improvement efforts to support organizational initiatives and enhance service delivery standards.

• Facilitating referrals and social work support services, guiding clients towards local resources such as medication assistance programs.

The patients served by Serving Children and Reaching Families span various demographics, including children, adolescents, adults, and families facing mental health challenges and substance use disorders. Qualifications\n

• A Master’s degree in Social Work from an accredited institution.
