Key Responsibilities\n

As a Licensed Clinical Social Worker at Patient Support Psychotherapy, you will:

• Directly engage with clients experiencing major mental illnesses or severe emotional disorders, providing compassionate and individualized therapeutic interventions both in virtual settings and through outreach.

• Conduct thorough evaluations and assessments, including brief, evidence-based screenings, and arrange further diagnostic assessments as necessary.

• Create, implement, and monitor personalized integrated care plans that ensure clients receive the comprehensive care they need, improving their access to linked services and enhancing communication among clients, family members, and healthcare providers.

• Support clients through a range of brief prevention, treatment, and recovery services tailored to their specific needs and circumstances.

• Carry and manage a caseload that includes individual, group, and family therapy, ensuring that all services align with the clients' presenting issues and requests.

• Document and track client progress, outcomes, and ongoing needs while ensuring timely completion of reports and compliance with regulatory requirements.

• Assist with crisis interventions and provide on-call support during and after regular program hours as assigned.

• Participate actively in team meetings and committee groups aimed at improving service delivery and quality improvement initiatives.

Qualifications\n

The ideal candidate will possess:
