Work Environment\n

This role operates within a vibrant community health clinic setting, which emphasizes remote support flexible scheduling. The work hours are generally Monday to Friday from 8:00 AM to 4:30 PM, with occasional evening and weekend hours as required to meet the needs of our clients. As a social worker, you will collaborate with healthcare professionals, caregivers, and family members to ensure that all aspects of our clients' well-being are addressed. Key Responsibilities\n

• Conduct thorough assessments of each senior's psychosocial needs, integrating emotional, mental, and social aspects into care plans.

Qualifications\n

• Master's degree in Social Work from an accredited institution.
