Job Details:

• Autonomously manage a patient panel of recently discharged and chronic acute exacerbation patients in an outpatient ambulatory clinic setting
• Perform remote monitoring of congestive heart failure patients
• Manage LVAD patients and collaborate closely with cardiologists
• Provide oversight and referrals for patient comorbidities
• Work with complex patients in an autonomous ambulatory setting
• Per diem position with available shifts Monday through Friday, 8am-4pm
• Must be eligible for licensure as an Advanced Practice Registered Nurse (APRN) in Connecticut
• Must hold or be eligible for certification as FNP, ANP, AGPCNP, ACNP, or AGACNP
• Prior experience as an Advanced Practitioner is required; congestive heart failure experience is highly preferred
• Access to professional development and wellbeing resources through a dedicated Office of Advanced Practice and Office of Provider Wellness

Compensation:
Competitive hourly rates with weekly pay and malpractice included.
