Key Responsibilities\n

• Conduct comprehensive psychosocial assessments to evaluate patients' emotional, social, and financial needs, ensuring a thorough understanding of each patient’s unique situation.

• Provide individual and group counseling sessions targeting issues such as adjustment disorders, grief, and coping mechanisms, tailored to the diverse backgrounds of our patient population.

• Collaborate actively with medical teams to create holistic discharge plans, connecting patients to community resources and services post-discharge to ensure successful outcomes.

• Offer emotional support and education to patients and their families, guiding them through various treatment options and available resources tailored to their specific needs.

• Participate in hospital committees and meetings, contributing your expertise on social work-related matters to enhance the organization's service delivery.

• Maintain accurate documentation consistent with hospital policies, ensuring compliance with legal and ethical standards of practice.

Patient Population / Client Focus\n

You will work with a diverse patient population, ranging from children to seniors, addressing mental health challenges and psychosocial issues that often accompany physical health concerns. Qualifications\n

• A Master’s Degree in Social Work from an accredited institution.
