Admissions Nurse RN

Avalon Care Center in Bellingham is seeking an Admissions Nurse to join our team!

$49.50 - $54.05 /hr

Part time: Saturday - Sunday

11:30 am - 7:00 pm

Placement within the wage range for this position is determined by years of experience, qualifications, and demonstrated ability to perform the essential functions.

Responsibilities

Admissions Process

• Complete admission assessments for incoming residents, including physical and clinical evaluations.
• Coordinate admissions to ensure a smooth transition into the facility.
• Review hospital records, physician orders, medications, and treatment plans prior to admission.
• Collaborate with physicians and interdisciplinary team members to support continuity of care.
• Ensure accurate and timely completion of admission documentation, authorizations, and medical records.
• Educate residents and families regarding admission procedures, care plans, and facility policies.
• Assess residents for appropriate level of care and identify immediate clinical or safety concerns.
• Coordinate room assignments, equipment, and resident care needs upon admission.
• Maintain compliance with state, federal, facility, and HIPAA regulations.
• Assist with resident transfers and discharge coordination as needed.

Qualifications

• Preferably 1-3 years of clinical nursing experience with at least two years in long term care and two years of successful experience in administration, consultation or involvement in the survey process

Licenses/Certification

• Must have an RN in good standing
