Key Responsibilities\n

• Conduct comprehensive assessments to accurately identify the needs and goals of patients.

• Regularly communicate progress with supervisors, families, and other involved parties to ensure continuity of care.

• Provide detailed scheduling of appointments, home visits, and monitoring of therapeutic interventions to ensure effective treatment.

• Coordinate in-home observations and chart reviews, utilizing data analysis to adjust treatment as needed.

• Ensure that the established program plans are implemented effectively and adapted based on patient responses.
\n
• Actively participate in interdisciplinary team meetings, contributing insights and strategies to enhance patient outcomes.

• Document clinical progress through detailed monthly and quarterly notes to track patient development.
\n
• Develop training materials to ensure that staff and team members are competent in program implementation and documentation standards.
\n
• Follow up with clients through interviews, home visits, and observations to provide ongoing support and assess evolving needs.

• Maintain meticulous records to ensure information is accessible to relevant stakeholders while adhering to legal and ethical standards.

• Serve as a liaison within the broader mental health community, working collaboratively to support developmental goals.

• Contribute to the continuous improvement of programs by assisting in the formulation and revision of training and procedural materials.

Patient Population / Client Focus\n

As a Licensed Professional Counselor at Adaptable Human Solutions Clinic, you will have the opportunity to work with diverse patient populations, including both adults and youth. You will address a range of mental health issues, from anxiety and depression to complex trauma and substance use disorders, tailoring your approach to meet the unique needs of each client.

Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field.

• Current and unrestricted LPC licensure in the state of Texas.
\n
• Minimum of two years of counseling experience, preferably within a clinical setting.

• Proficiency with electronic medical records (EMR) systems and familiarity with telehealth platforms.
\n
• Strong interpersonal skills with the ability to build rapport with a diverse range of clients.

• Ability to work collaboratively within a healthcare team, contributing collectively to patient care.

Preferred Experience\n

• Specialization in family therapy, couples therapy, trauma-informed care, or substance misuse counseling is highly preferred.

• Demonstrated understanding of legal and ethical standards in counseling practice.

Compensation and Benefits\n
