Key Responsibilities\n

• Conduct comprehensive psychosocial assessments to evaluate emotional, social, and environmental conditions impacting patients and their families.

• Design, implement, and monitor tailored treatment plans utilizing evidence-based interventions tailored to the unique needs of each patient.
\n
• Collaborate closely with a multidisciplinary team including physicians, nurses, and addiction specialists to facilitate holistic care for patients.
\n
• Guide and support patients through the discharge planning process, ensuring continuity of care, and assist in linking them to community resources.
\n
• Provide ongoing education and counseling to patients and families regarding adaptation strategies to manage illness and recovery.
\n
• Administer various therapeutic modalities—crisis intervention, brief and long-term individual therapies, group sessions, or family therapy, as clinically appropriate.
\n
• Participate in an on-call rotation to respond to after-hours situations, addressing patient needs promptly and effectively.
\n
• Evaluate and support oncology patients experiencing mental health issues such as depression or anxiety, delivering short-term counseling when indicated.
\n
• Facilitate involuntary examinations under the Baker Act if clinically necessary, ensuring patient safety and compliance with legal protocols.
\n
• Engage in community outreach efforts, providing educational workshops and informational resources to enhance community support services for cancer survivors.
\n
• Serve as a mentor and resource for colleagues, offering guidance on psychosocial oncology practice and suggesting best practices.

Patient Population / Client Focus\n

This role primarily focuses on patients in the detoxification phase, including individuals dealing with substance use disorders and those in transitional stages of mental health recovery. The Social Worker will also work with families to address the broader impact of addiction and mental health issues, ensuring an inclusive approach that enhances treatment outcomes.

Qualifications\n

• Master's degree in Social Work from an accredited institution.

• Active and unrestricted LCSW license in Texas.

• Strong understanding of mental health disorders, treatment planning, and the principles of addiction recovery.

• Demonstrated ability to work collaboratively within a multidisciplinary team.
