Care Coordinator

Role Overview: The Care Coordinator supports children aged 0–3 with developmental delays or disabilities and their families through the Birth to Three Program. Key Responsibilities: Conduct assessments, develop and implement family-centered service plans, coordinate services, provide family education and advocacy, maintain documentation, facilitate team meetings, collaborate with community providers, and adapt schedules to meet client needs. Qualifications: Bachelor's degree in Social Work, Child Development, or related fields; experience working with families with special needs preferred. Valid driver's license, reliable transportation, strong communication, organizational skills, and ability to work in various settings are required.
