Key Responsibilities\n

• Coordinate with Reentry Social Workers to ensure that discharge planning services are appropriately tailored to individual client needs.

• Complete necessary statistical reports and documentation in compliance with organizational standards and regulations.

• Collaborate effectively with the Department of Correction to manage patient access, recommend suitable housing, and determine treatment paths.
\n
• Lead and develop group therapy sessions, particularly in mental health observation units, fostering a supportive environment for participants.
\n
• Conduct mental health evaluations, crisis intervention strategies, assessments, and follow-ups, focusing on recovery and stabilization.
\n
• Engage in weekly clinical supervision to enhance clinical practice and stay updated on relevant methodologies.

• Facilitate case conferences and contribute to treatment planning for patients both currently in treatment and those being considered for care.

• Potentially supervise social work interns or student volunteers, providing mentorship to the next generation of mental health professionals.

• Conduct psychological assessments and relevant testing to inform treatment strategies.

• Determine and coordinate appropriate levels of mental health care, ensuring clients receive personalized and effective support.
