Key Responsibilities\n

As a Licensed Clinical Social Worker (LCSW), you will engage in a variety of critical responsibilities including:

• Conducting comprehensive psychosocial assessments to identify patients' emotional, social, spiritual, and financial needs during their hospice journey.

• Providing individual and group therapy sessions focused on coping strategies, adjustment to illness, grief, and loss, helping patients and families navigate their unique challenges.

• Coordinating care with an interdisciplinary team to develop individualized care plans that address both medical and psychosocial aspects of patient well-being.

• Facilitating communication between families and care teams to ensure all parties are informed and involved in care decisions.

• Identifying and connecting patients with community resources and support services for ongoing assistance post-discharge.

• Maintaining thorough and accurate documentation in alignment with hospital protocols and regulatory standards, ensuring continuity and quality of care.

• Providing crisis intervention as needed, ensuring that immediate emotional support is readily available for patients and their families.

Patient Population / Client Focus\n

The primary focus of this role is the hospice patient population, which encompasses individuals with terminal illnesses and their families. You will play a pivotal role in empowering patients to achieve the highest possible quality of life through symptom management and emotional support while offering guidance through the grieving process to their loved ones.

Qualifications\n

The ideal candidate will possess the following qualifications:

• A Master’s degree in Social Work from an accredited program.

• An active state licensure as a Licensed Clinical Social Worker (LCSW).

• Proficiency in understanding medical terminology, mental health disorders, and basic crisis intervention techniques.

• Strong communication and interpersonal skills, with the ability to build rapport quickly and effectively with patients and families.

Preferred Experience\n

Compensation and Benefits\n
