Work Environment

This position is primarily onsite, delivering care directly in patients' homes. Working as part of a dynamic interdisciplinary team, you'll be involved in comprehensive patient assessments and care planning. Key Responsibilities

• Conduct thorough evaluations of patients to assess physical abilities, limitations, and rehabilitation needs, followed by the formulation of personalized treatment plans based on physician orders.
• Implement therapeutic interventions, including manual therapy, modalities, exercise instruction, and patient education, to facilitate recovery and promote mobility improvement.
• Maintain accurate and timely documentation regarding patient evaluations, treatments, progress notes, and discharge plans in accordance with regulatory and organizational standards.
• Provide supervision to Physical Therapist Assistants (PTAs) and support staff, guiding them to achieve optimal patient outcomes.
• Regularly collaborate with an interdisciplinary team including physicians, nurses, occupational therapists, and social workers to ensure comprehensive patient care and continuity of services.
• Evaluate and adjust treatment plans as necessary, monitoring patient responses to interventions and making data-driven decisions.
• Participate in professional development activities and contribute feedback for enhancing departmental goals and productivity.

Patient Population / Client Focus

You will cater to a diverse patient population, including individuals recovering from surgery, those with chronic conditions, and seniors requiring rehabilitation services. Your role will focus on creating individualized treatment plans that accommodate the unique needs of each patient, thereby contributing to their recovery journey and overall quality of life.

Qualifications

• Bachelor's Degree in Physical Therapy from an accredited program.
• Current Physical Therapist license in the State of New Jersey.
• Strong verbal and written communication skills to effectively interact with patients and healthcare team members.
• Basic computer proficiency, including experience with electronic health records (EHR) and documentation software.

Preferred Experience

• A minimum of one year of direct experience in a physical therapy setting, preferably in home care, is advantageous.
• Experience using evidence-based practices to guide treatment decisions and enhance patient outcomes.

Compensation and Benefits
