Work Environment

This position is onsite and fully focused on home care, providing you with the unique opportunity to create one-on-one, personalized rehabilitation experiences for your patients in their own surroundings. Key Responsibilities

• Conduct thorough evaluations, including reviewing patient history and performing specific assessments to formulate an evidence-based treatment plan.
• Communicate evaluation findings and treatment recommendations clearly to patients and their families, incorporating their feedback into goal-setting activities.
• Implement treatment protocols grounded in the latest healthcare practices and adjust based on patient response and evolving health status.
• Consult and coordinate with multidisciplinary healthcare professionals as necessary, ensuring comprehensive care and timely adjustments to treatment plans based on patient progress.
• Develop and complete discharge plans, providing detailed, accurate documentation of patient interactions while maintaining compliance with evolving healthcare regulations.
• Exhibit effective time management skills to meet productivity standards and maintain accurate scheduling for thorough patient follow-ups.
• Engage in team initiatives to enhance patient access to care, actively participating in marketing and outreach programs within the community.

Patient Population / Client Focus

You will primarily be serving the local community of Deltona and surrounding areas, helping patients recover from surgeries, manage chronic conditions, or rehabilitate after injuries. The caseload involves a variety of conditions, ensuring a rich and rewarding professional experience as you make significant impacts on the quality of life for individuals of all ages.

Qualifications

• Bachelor's degree in Physical Therapy from an accredited institution.
• Valid state license to practice as a Physical Therapist in Florida.
• Current BLS certification for Healthcare Providers from the American Heart Association.

Preferred Experience

• A minimum of 2 years of experience in a physical therapy setting, with a strong preference for home care experience.

Compensation and Benefits
