OnTrack Physical Therapy & Wellness is actively seeking a dedicated and experienced Licensed Physical Therapist (PT) to join our team in Sanford, Florida. Work Environment

This position is specifically onsite in the homes of patients, providing a dynamic and personalized work environment unlike a traditional clinic setting. Our physical therapists take pride in visiting patients in their own spaces, which allows for a comprehensive understanding of their daily challenges and better alignment with their rehabilitation goals. You will work Monday through Friday from 8:00 am to 5:00 pm, creating a steady and predictable schedule that helps maintain a balanced professional and personal life.

Key Responsibilities

• Conduct thorough assessments and evaluations of patients with functional limitations and disabilities, developing tailored treatment plans that outline achievable goals and therapeutic strategies.
• Implement and monitor treatment protocols based on evaluation findings, adapting strategies as necessary to ensure optimal patient progress.
• Engage with other healthcare professionals to coordinate care, recognizing changes in patient conditions that necessitate adjustments in treatment plans or referrals for further medical consultation.
• Document all patient interactions accurately and in a timely manner, ensuring compliance with evolving healthcare regulations and providing a clear account of each patient's treatment journey.
• Facilitate proper discharge planning and follow-up care, making recommendations that support continued rehabilitation and health beyond therapy sessions.
• Actively participate in training sessions, corporate meetings, and initiatives aimed at improving patient access and outcomes.
• Maintain an updated understanding of relevant state practice acts and corporate compliance standards.

Patient Population / Client Focus

As a Physical Therapist at OnTrack, you will be working closely with a wide variety of patients, including those recovering from surgery, managing chronic illnesses, or coping with age-related challenges. Your focus will be on developing meaningful relationships with each patient, understanding their individual circumstances, and applying your expert knowledge to help them regain mobility and enhance their quality of life.

Qualifications

• A Doctorate in Physical Therapy (DPT) from an accredited institution.
• Current state licensure as a Physical Therapist by the Physical Therapy Examining Committee.
• A minimum of two years of experience in an outpatient orthopedic setting.
• A valid CPR certification to ensure safety and readiness in emergency situations.

Preferred Experience

While not mandatory, experience in home health care and familiarity with electronic medical records (EMR) systems would be advantageous, promoting your ability to manage patient data efficiently and improving overall workflow.

Compensation and Benefits

We offer a competitive annual salary ranging from $100,000 to $120,000, commensurate with experience and qualifications.
