This role is based in Sanford, Florida, where you will operate in a flexible work environment directly within patients' homes. Key Responsibilities

• Conduct thorough evaluations of patients' physical conditions, including detailed assessments and re-evaluations, interpreting results within the scope of your licensure.
• Develop and implement individualized therapy plans that prioritize early intervention and promote effective outcomes.
• Collaborate with other healthcare professionals within an interdisciplinary team to review patient histories, address clinical needs, and refine treatment strategies.
• Manage clinical outcomes effectively, addressing complex cases and concerns with staff therapists to foster continuous improvement.
• Oversee clinical workflows to maximize patient engagement, ensuring seamless processing of referrals, evaluations, and therapy session scheduling.
• Maintain accurate documentation and perform comprehensive chart reviews to uphold regulatory and ethical standards.

Patient Population / Client Focus

The primary focus will be on a varied patient demographic that may include elderly individuals recovering from surgery, patients with chronic pain, and those undergoing rehabilitation after injury. Qualifications

• Active Florida State Licensure as a Physical Therapist.

Compensation and Benefits
