Work Environment

This full-time position requires working on-site, primarily within patients' homes in Sanford and the surrounding areas. You will engage with clients one-on-one in their home environments, allowing you to assess and address their specific therapeutic needs. Key Responsibilities

• Direct physical therapy treatment focused on preventing disability, relieving pain, and restoring muscle function in accordance with the established plan of care.
• Supervise and mentor physical therapy assistants to ensure consistent and effective implementation of therapeutic strategies.
• Conduct comprehensive evaluations to assess patients' functional abilities, utilizing diagnostic testing and assessments.
• Analyze home environments to determine the most beneficial adaptations that could increase clients' independence and enhance rehabilitation outcomes.
• Provide skilled physical therapy interventions as prescribed by physicians, performing manual therapeutic exercises, and instructing clients on functional activities.
• Administer various therapeutic techniques, including traction and massage, while applying physical agents as needed.
• Evaluate, fit, and make recommendations regarding prosthetic and orthotic devices to optimize patient mobility.
• Document all patient interactions and treatment plans comprehensively by preparing initial evaluations, progress notes, and discharge summaries.
• Collaborate with an interdisciplinary team during case conferences to discuss, plan, and evaluate collective treatment strategies for clients.
• Educate clients and families on using physical therapy devices and understanding their home therapy regimens.
• Ensure timely documentation and adherence to policies concerning clinical records and payroll submissions.
• Maintain accessibility during PT Assistant treatments to provide support or intervention as needed.
• Engage in ongoing evaluations and revisions of care plans integrating community resources effectively.

Patient Population / Client Focus

The patients you will support range from those recovering from surgeries to individuals managing chronic conditions, aged both young and elderly. Your work will directly enhance their functionality and well-being in daily activities, accumulating towards their overall rehabilitation goals. Qualifications

• Completion of a Doctorate in Physical Therapy (DPT) from an accredited program.
• An active Physical Therapist license in the state of Florida, free from sanctions or board proceedings.
• Profound expertise in manual therapy modalities accompanied by a visible success record in implementing exercise programs.
• Familiarity with EMR systems, particularly Web PT, for efficient and precise documentation of patient records.
• In-depth knowledge of contemporary physical therapy techniques backed by current research.

Preferred Experience

• A strong background in home health care settings, enabling a nuanced understanding of in-home patient needs.
• Experience working collaboratively within a multidisciplinary team.

Compensation and Benefits

The annual salary for this role ranges from $100,000 to $120,000, supplemented by a comprehensive benefits package for full-time team members.
