Key Responsibilities

• Conduct comprehensive evaluations and assessments of patients, identifying functional limitations and creating tailored treatment plans to facilitate recovery and enhance mobility.
• Implement effective therapeutic interventions that adhere to evidence-based practices, adjusting treatment protocols based on patient progress and feedback.
• Educate patients and their families on treatment plans and exercises, empowering them to take an active role in their recovery process.
• Collaborate within an interdisciplinary treatment team, maintaining open communication with other healthcare providers, referring physicians, and community organizations to optimize patient care.
• Maintain accurate and timely documentation of patient evaluations, progress notes, and treatment plans in accordance with legal and professional standards.
• Remain engaged in the community through outreach programs, educational workshops, and involvement in local health initiatives to promote awareness and accessibility of physical therapy services.
• Participate in professional development meetings and educational programs to enhance your clinical skills and stay current with best practices.

Patient Population / Client Focus

The ideal candidate will work with a diverse group of patients, primarily in orthopedic recovery, including those recovering from surgery, injury, or managing chronic pain. Our patient population also includes older adults seeking to maintain their functionality and quality of life. You will address a wide array of needs and conditions, requiring a compassionate and adaptive approach to therapy.

Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited institution.
• Active State License in Physical Therapy.
• Basic Life Support (BLS) certification.
• Strong verbal and written communication skills, with proficiency in word processing and electronic medical record systems.

Preferred Experience

A minimum of two years of clinical experience in a physical therapy setting is required. Experience in home health care and familiarity with orthopedic conditions will be advantageous. Candidates with a proven ability to work collaboratively within a team-oriented environment will excel in this role.

Compensation and Benefits
