Work Environment

This role is based entirely onsite, providing home care services in the Columbia area. You will work in a collaborative environment that fosters support, communication, and continuous learning among healthcare professionals. The average workweek is approximately 40 hours, with scheduled shifts typically from 9:00 a.m. to 6:00 p.m., and occasional evening clinics until 8:00 p.m. Participation in community outreach events and health fairs may require weekend availability. This setting allows for a direct impact on patient care and the overall health of the community.

Key Responsibilities

• Perform thorough health assessments and screenings for patients of all ages, identifying both acute and chronic conditions.
• Formulate diagnostic strategies and develop personalized treatment plans, ensuring an emphasis on preventive care and health promotion.
• Collaborate with community organizations and health agencies to design and deliver educational programs that enhance community health awareness.
• Provide culturally competent care that respects diverse values and beliefs while advocating for patients’ rights to access necessary healthcare services.
• Manage patient referrals and coordinate follow-up care to maintain continuity of care and improve health outcomes.
• Document all patient interactions comprehensively in compliance with legal and organizational standards, utilizing our electronic health record system.
• Engage actively in community outreach efforts, including health fairs and informational workshops, to improve public health literacy.
• Stay knowledgeable about emerging health trends, evidence-based practices, and legislative changes affecting healthcare delivery.

Patient Population / Client Focus

The Community Health Nurse Practitioner will serve a diverse population, including individuals and families from various cultural, economic, and social backgrounds. You will address a range of health concerns, focusing on preventive care, chronic disease management, and health education, with an emphasis on vulnerable communities who experience barriers to care.

Qualifications

• A Master’s degree in Nursing from an accredited institution.
• Current certification as a Nurse Practitioner with an active state nursing license.
• A minimum of 3-5 years of clinical experience, ideally within community health or public health environments.
• Exceptional interpersonal and communication skills to effectively connect with patients and collaborate with multidisciplinary teams.
• A strong commitment to social justice and the ability to advocate for marginalized populations.

Preferred Experience

• Experience in a home care setting or community-based organization.
• Knowledge of local health resources and community services available in Columbia, MD.

Compensation and Benefits

• We offer a competitive salary range of USD 120,000 to 140,000 annually, based on experience and qualifications.
