Key Responsibilities\n

• Conduct thorough ongoing assessments to understand and address the individual needs of each patient.

• Maintain open communication with supervisors, healthcare staff, family members, and relevant parties to ensure continuity of care and keep stakeholders informed about patient progress.

• Develop and present a structured appointment schedule to supervisors involving necessary home visits and observations.
\n
• Coordinate home visits and ensure compliance with in-home observations, including chart reviews and staff interviews to facilitate comprehensive care.
\n
• Monitor and evaluate the effectiveness of program implementation and make adjustments as needed to align with treatment goals.
\n
• Participate in interdisciplinary team meetings and key discussions to contribute to strategic care decisions.
\n
• Document monthly and quarterly progress notes, ensuring accurate and timely communication with the healthcare team.
\n
• Organize and facilitate training sessions for team members, ensuring compliance with best practices in treatment and documentation standards.
\n
• Conduct follow-up interviews and home visits to provide ongoing support tailored to individual patient needs.
\n
• Maintain comprehensive records, ensuring that information is readily available to pertinent stakeholders.
\n
• Serve as a liaison between the mental health community and the public to foster understanding and support mental wellness initiatives.

• Assist in the creation and refinement of training materials to enhance therapeutic programs offered to clients.

Patient Population / Client Focus\n

In this role, you will serve a diverse patient population that includes both adults and youth facing a range of mental health challenges. Your expertise in counseling will guide clients through their individual struggles, with a particular focus on issues such as family dynamics, trauma, and substance misuse. You will be instrumental in facilitating a safe and supportive environment where clients feel comfortable expressing their concerns and working towards their goals.

Qualifications\n

• A Master's or Doctoral degree in Counseling, Psychology, or a closely related field is required.

• An active and unrestricted licensure as a Professional Counselor (LPC) in the state of Texas.

• At least two years of experience in a counseling setting, preferably in a clinical context.

• A strong understanding of legal and ethical standards within the mental health field.

• Outstanding interpersonal and communication skills, with the ability to establish rapport with a diverse clientele.

Preferred Experience\n

Compensation and Benefits\n
