Key Responsibilities\n

• Conduct thorough mental health assessments for patients, taking into account their medical history, current diagnoses, and emotional states.

• Develop and implement individualized care plans in collaboration with interdisciplinary team members, focusing on enhancing the emotional and psychological well-being of patients and their families.

• Provide therapeutic interventions, including evidence-based practices such as cognitive-behavioral therapy (CBT) and supportive counseling, tailored to the unique needs of hospice patients.

• Facilitate individual and family therapy sessions aimed at helping patients and their loved ones process grief and loss and develop effective coping strategies.

• Coordinate care with medical professionals, including physicians and nurse practitioners, to ensure a comprehensive approach to each patient's care.

• Proactively provide crisis intervention and immediate support during emergencies involving patients and families, utilizing de-escalation techniques and making appropriate referrals when necessary.

• Offer education and resources to families about grief, mental health conditions, and the available support systems within and outside the hospice care framework.

• Maintain accurate and timely documentation of all patient interactions, treatment plans, and clinical notes in compliance with state regulations and ethical standards.

Qualifications\n

• Master's degree in Social Work from an accredited institution.
