Key Responsibilities\n

• Engage with insurance providers for pre-certifications and manage utilization, streamlining access to necessary services for our clients.

• Play an essential role in discharge planning and aftercare services, ensuring that our clients transition smoothly from hospice care to home or other care settings.

Patient Population / Client Focus\n

You will primarily serve older adults experiencing significant life transitions, including terminal illness and end-of-life situations. Compensation and Benefits\n

The annual salary for this position ranges from $90,000 to $105,000, commensurate with experience and qualifications.
