Key Responsibilities\n

• Conduct comprehensive assessments of the psychosocial needs of clients particularly focusing on seniors' emotional, mental, and social aspects.

• Collaborate with clients and their families to create personalized care plans that align with their specific goals and preferences, ensuring a client-centered approach to care.

• Provide counseling and emotional support, assisting clients and their families in navigating challenges associated with life transitions.

• Act as an advocate for clients' rights, facilitating access to necessary community resources and services that enhance their quality of life.

• Engage with interdisciplinary teams, including nurses and healthcare professionals, to seamlessly coordinate and elevate client care.

• Monitor clients' well-being through regular check-ins, assessing living conditions and addressing any safety concerns.

• Facilitate open communication among family members involved in care, offering conflict resolution and support.
\n
• Educate families about support groups and community resources available to improve engagement and social interactions.

• Maintain accurate documentation of client assessments, care plans, progress notes, and interactions to ensure compliance and continuity of care.

Patient Population / Client Focus\n

You will primarily work with seniors in the hospice setting who experience a range of emotional and psychosocial issues associated with aging, illness, and end-of-life transitions. Qualifications\n

• Master's degree in Social Work from an accredited institution.

• Current and valid state licensure as a Licensed Clinical Social Worker (LCSW).
