Work Environment\n

This position allows for remote work flexibility, enabling you to provide care from the comfort of your home. However, the candidate must be able to occasionally visit our hospice center in San Francisco for client assessments or team meetings as needed. Key Responsibilities\n

As a Licensed Clinical Social Worker at Riverbend, your day-to-day responsibilities will include:

• Conducting comprehensive psychosocial assessments to identify clients' mental health concerns, personal strengths, and available resources.

• Providing individual, family, and group therapy utilizing evidence-based treatment modalities such as cognitive-behavioral therapy (CBT), solution-focused therapy, and trauma-informed care.

• Developing personalized treatment plans that align with clients' goals and demonstrate evidence-based interventions.

• Facilitating group therapy sessions, psychoeducation classes, and support groups to enhance community engagement and understanding.

• Collaborating with a multidisciplinary team, including psychiatrists and psychologists, to ensure coordinated and holistic care for clients.

• Offering crisis intervention, risk assessments, and safety planning as necessary, particularly for high-risk clients.

• Maintaining precise and timely documentation in accordance with ethical and legal standards, utilizing electronic health record systems.

This role focuses primarily on clients facing significant health challenges, including terminal illnesses and complex emotional distress associated with end-of-life issues. Qualifications\n

To be considered for this role, candidates must possess the following qualifications:
