Key Responsibilities\n

You will play a critical role in delivering high-quality mental health services, including:

• Engaging directly with clients both in the office (limited) and the community, conducting assessments, and identifying individual needs.
\n
• Collaborating with a multidisciplinary team to develop and implement comprehensive treatment plans aimed at addressing the unique challenges faced by clients.
\n
• Facilitating individual, group, and family therapy sessions to promote effective coping strategies and support.
\n
• Utilizing evidence-based practices to perform thorough screening and assessments, ensuring that clients receive appropriate care and resources.

• Providing crisis intervention services and support as necessary, ensuring timely and effective responses to urgent client needs.
\n
• Documenting all interactions, treatment plans, and progress notes in compliance with ethical standards and organizational guidelines.

• Participating actively in team meetings and committees to continuously improve service delivery and client outcomes.

Patient Population / Client Focus\n

Your services will primarily focus on a diverse clientele who experience major mental health issues and severe emotional disorders, particularly within the hospice care setting. You will work with both patients and their families, addressing their emotional, psychological, and social needs during a particularly vulnerable time in their lives.

Qualifications\n

To be considered for this position, applicants must meet the following criteria:

• Current licensure as a Clinical Social Worker (LCSW) in New York.

• A Bachelor’s Degree in Social Work or a related field is required; advanced degrees are preferred.

• A minimum of two years of clinical experience, specifically within a mental health or hospice care environment.

• Strong ability to engage and build rapport with clients from diverse backgrounds, demonstrating empathy and respect.

• Familiarity with electronic health records (EHR) and other clinical documentation systems.

Compensation and Benefits\n

This full-time position offers a competitive salary range of USD 70,000–85,000 annually, commensurate with experience and qualifications.
