Key Responsibilities\n

• Collaborate closely with healthcare professionals to develop comprehensive treatment plans and discharge strategies based on thorough psychosocial assessments and established social work interventions.

• Conduct timely assessments and interventions tailored to patients' needs, ensuring detailed documentation including clinical notes and progress reports compliant with our department policies.

• Provide compassionate counseling and crisis intervention, addressing the emotional and behavioral challenges faced by patients coping with end-of-life issues.

• Engage with families and support systems, facilitating discussions around client treatment options, discharge planning, and available community resources.

Patient Population / Client Focus\n

Your focus will be on individuals in hospice care, dealing with serious illnesses and end-of-life concerns. This demographic often experiences complex emotional and psychological needs, requiring a sensitive and informed approach. You will work not only with patients but also with their families, addressing grief processes, emotional distress, and the psychosocial implications of terminal diagnoses.

Qualifications\n

• A Master’s degree in Social Work or Counseling from an accredited program.

• Active and unrestricted LICSW licensure in the state of Washington.

• At least 3 years of unsupervised, post-licensure experience in clinical social work.

• Strong computer proficiency, including familiarity with electronic medical records, word processing, and spreadsheet applications.

Preferred Experience\n

While the minimum requirement is a bachelor’s degree, candidates possessing a Master’s degree will be viewed favorably. Compensation and Benefits\n
