This position is primarily remote, allowing for flexibility while still ensuring direct interaction with clients. Key Responsibilities\n

• Perform detailed assessments of seniors' psychosocial needs, evaluating emotional and social factors that impact their well-being.

• Develop and implement personalized care plans in collaboration with clients and their families, aligning with their specific goals and preferences.

• Provide counseling and emotional support, guiding clients and their families through complex life transitions, including grief and loss.

• Advocate for clients’ rights to accessing community resources, ensuring they receive necessary support and services during this critical period.

• Work collaboratively with an interdisciplinary team comprising nurses, caregivers, and healthcare professionals to facilitate seamless transitions of care.

• Conduct regular home visits to monitor clients' conditions, ensuring safety and addressing any concerns regarding their living situations.

• Assist in mediation and conflict resolution among family members involved in the care of the senior client.

• Educate clients and their families regarding available resources, support groups, and community programs that enhance social engagement and quality of life.

• Maintain accurate and thorough documentation of all client assessments, care plans, progress notes, and relevant interactions following regulatory standards.

Qualifications\n

Compensation and Benefits\n

The salary range for this position is competitive, ranging from USD 70,000 to 100,000 annually, commensurate with experience and qualifications.
