Key Responsibilities\n

• Assist patients in navigating community resources, such as vocational rehabilitation, housing assistance, and support groups, enhancing their connectedness post-rehabilitation.

• Educate patients and families on available community resources, post-discharge services, and advocacy techniques to support their ongoing needs after rehabilitation.

• Participate in discharge planning meetings, ensuring smooth transitions for patients to home or other appropriate care settings while addressing any lingering psychosocial concerns.

Patient Population / Client Focus\n

You will primarily work with individuals facing rehabilitation challenges stemming from conditions such as stroke, spinal cord injuries, orthopedic surgeries, and various neurological disorders. The focus will be on understanding and addressing the emotional and social barriers that impact their recovery, enabling the transition back into their communities as empowered individuals.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• Active licensure as a Licensed Clinical Social Worker (LCSW) in the state of Kansas.
