Key Responsibilities\n

• Conduct thorough psychosocial assessments through interviews and meticulous review of medical records to evaluate emotional and social factors affecting patient recovery.

• Assist patients in identifying and accessing community resources such as vocational rehabilitation, support groups, and housing assistance, ensuring they have the tools to support their recovery.

• Educate patients and their families about available community resources and post-rehabilitation services to facilitate seamless transitions into everyday life.

• Take an active role in discharge planning, providing essential guidance to patients as they prepare to return home or transition to other care settings.

Patient Population / Client Focus\n

In this position, you will primarily work with a diverse range of patients recovering from conditions such as stroke, spinal cord injuries, orthopedic surgeries, and neurological disorders. Qualifications\n

Compensation and Benefits\n
