Key Responsibilities\n

• Conduct in-depth psychosocial assessments through interviews, observations, and medical record reviews to diagnose and understand the emotional, social, and environmental factors influencing each patient's recovery journey.

• Provide compassionate counseling and emotional support to patients and their families, helping them navigate adjustment issues, mental health challenges, and understanding their rehabilitation process.

• Collaborate actively with the rehabilitation team to formulate and implement tailored treatment plans that address both psychosocial and clinical needs.

• Guide patients in accessing community resources such as support groups, vocational rehabilitation services, and housing assistance, facilitating their successful reintegration into the community.

• Educate patients and their families about post-rehabilitation services, ensuring they have the information needed to continue their journey towards independence.

• Participate in discharge planning processes, ensuring seamless transitions from rehabilitation to home or alternative care settings, which is critical to sustained patient recovery and satisfaction.

Patient Population / Client Focus\n

The Licensed Clinical Social Worker will work primarily with a diverse group of patients undergoing rehabilitation for various conditions, including neurological disorders, orthopedic surgeries, and recovery from strokes and spinal cord injuries. By addressing their emotional and practical needs, you will play a crucial role in enhancing their rehabilitation experience and overall outcomes.

Qualifications\n

• A Master’s degree in Social Work from an accredited program.

• Current active licensure as a Licensed Clinical Social Worker (LCSW) in California.

Compensation and Benefits\n
