Key Responsibilities\n

• Conduct comprehensive intake assessments via secure video conferencing to evaluate new members' symptoms, functioning, and daily living habits.

• Provide ongoing virtual support, including psychoeducation, real-time encouragement, and adjustments to treatment plans based on client needs and progress.

• Utilize digital tools to monitor member engagement and clinical assessments to refine and optimize care plans continuously.

• Celebrate client successes, addressing barriers to engagement collaboratively, and encouraging perseverance through challenges.

• Maintain accurate documentation of all client interactions and clinical records, adhering to professional and organizational standards.

• Work collaboratively with psychiatrists, fellow social workers, and other healthcare professionals to ensure a seamless integration of care across disciplines.

Patient Population / Client Focus\n

The primary client base for this role includes individuals in hospice care, experiencing complex emotional and psychological challenges as they navigate end-of-life transitions. Qualifications\n

• A Master’s degree or Doctorate in Social Work, with a current and valid state license in good standing.

Compensation and Benefits\n
