Key Responsibilities\n

• Engage directly with clients, both in virtual and community settings, guiding them through their mental health challenges associated with terminal illness or severe emotional disorders.

• Facilitate initial assessments through evidence-based screenings to determine clients' needs and create tailored care plans that link them to necessary services.

• Develop and implement integrated treatment plans through collaboration with clients, families, and other healthcare providers, ensuring continuity and accessibility of care.

• Provide crisis intervention when required, utilizing de-escalation techniques and therapeutic communication to support clients in distress.

• Maintain clear and timely documentation of client interactions, treatment progress, and care coordination as per organizational standards and compliance requirements.

• Participate in team meetings and committees aimed at enhancing collaboration, improving service delivery, and addressing agency quality improvement initiatives.

• Support clients through a range of services, including individual, group, and family therapy, ensuring a client-centered approach at all times.

Compensation and Benefits\n

The salary for this position ranges from USD $70,000 to $85,000 annually, commensurate with experience.
