This full-time role will primarily support clients at our remote Hospice Center in Hialeah, Florida. Key Responsibilities\n

• Directly engage with assigned clients to conduct assessments, individual, group, and family therapy addressing major mental illnesses and severe emotional disorders.

• Utilize evidence-based practice to conduct brief, developmental screenings and arrange for comprehensive assessments as necessary.

• Create and implement integrated care plans, ensuring clients have access to necessary services and resources, fostering effective communication among families and providers.

• Facilitate crisis intervention efforts, intervening when necessary to maintain client safety and well-being.
\n
• Monitor client admissions, continued stays, and discharge criteria, ensuring timely and appropriate treatment services are provided based on client needs and requests.

• Maintain accurate and timely documentation in compliance with organizational standards, regulatory requirements, and ethical guidelines.

• Participate in team meetings and committees focused on improving services across the agency, reinforcing collaboration and shared goals.

• Provide on-call support during and after program hours as assigned, demonstrating commitment to ongoing client support.

This role primarily serves clients facing significant mental health challenges within hospice care. Qualifications\n

• Master’s degree in Social Work from an accredited institution.

Compensation and Benefits\n

This position offers a competitive salary range of USD 70,000 – 85,000 per year, commensurate with experience and qualifications.
