Key Responsibilities\n

As an LCSW at the American Counseling Center, your core responsibilities will include:

• Providing tailored counseling and psychological support to patients and their families, focusing on adjustment issues and emotional well-being.

• Collaborating with other rehabilitation professionals to create individualized treatment plans that address the psychosocial needs of patients.

• Assisting patients in navigating and accessing community resources, such as support groups, vocational rehabilitation services, and housing assistance, to enhance their quality of life.

• Educating patients and their families regarding post-rehabilitation services and community resources to ensure a smooth transition to their home environment.

• Contributing to discharge planning to facilitate a safe and effective return home, coordinating with other healthcare providers as needed.

Patient Population / Client Focus\n

You will work closely with a diverse group of patients, primarily those facing end-of-life challenges and complex rehabilitation needs. Qualifications\n

The successful candidate must possess the following qualifications:
