This position is fully remote, providing the flexibility to work from various locations while still engaging with a collaborative team of rehabilitation professionals. The role may occasionally involve attending in-person meetings at our hospice center, allowing for a dynamic blend of remote and on-site collaboration as needed.

Key Responsibilities\n

• Conduct thorough psychosocial assessments by interviewing patients, observing behaviors, and reviewing medical records to evaluate emotional, social, and environmental factors that could impact recovery.

• Provide empathetic counseling and support to patients and their families, helping them navigate adjustment challenges related to illness or disability.

• Collaborate with an interdisciplinary team, including physicians, physical therapists, and occupational therapists, to create personalized treatment plans that factor in patients' psychosocial needs.

• Assist patients in accessing valuable community resources such as support groups, vocational rehabilitation, or housing assistance, essential for their rehabilitation journey.

• Educate patients and families about available community resources and post-rehabilitation services, ensuring smooth transitions to independence after treatment.

• Participate proactively in discharge planning, facilitating effective transitions for patients as they move from our care to home or other care settings.

Patient Population / Client Focus\n

The LCSW will engage with a wide range of patients who may have experienced strokes, spinal cord injuries, orthopedic surgeries, and various neurological disorders. Compensation and Benefits\n
