Key Responsibilities\n

• Conduct in-depth evaluations and assessments of children and their family dynamics, identifying strengths and areas for growth.

• Participate actively in the development and implementation of Individualized Family Service Plans (IFSPs).
\n
• Monitor the outcomes of services provided, making adjustments to interventions as necessary in order to meet client needs effectively.

• Offer guidance and support to families, empowering them with strategies and resources to enhance their child's development and well-being.
\n
• Ensure compliance with all federal and state regulations regarding social work practices and patient confidentiality.
\n
• Develop rapport with children and families, fostering a trusting relationship that supports a healthier learning environment.

• Utilize evidence-based practices and natural learning environments to instruct families in effective interventions.
\n
• Conduct coaching sessions in home or community settings, focusing on practical strategies for daily interactions and routines.

• Work collaboratively with a multidisciplinary team, contributing your expertise to enhance overall service delivery.

• Maintain accurate and timely documentation of services provided, following up on appointments and assessing ongoing needs.

Patient Population / Client Focus\n

You will primarily work with children aged 0 to 3 years who exhibit disabilities or developmental delays, as well as their families. The focus is on delivering early intervention services tailored to the individual needs of each child, aligning with their family’s values and priorities. Your role is critical in helping families navigate challenges while celebrating progress and milestones in their child’s development.

Qualifications\n

• Master's Degree or Doctorate in Social Work from an accredited institution.

• Active licensure from the Department of Public Health as a Licensed Clinical Social Worker (LCSW).

• A valid driver's license, with the ability to travel locally for client visits when necessary.

• Excellent organizational skills paired with the ability to manage time effectively in a remote work setting.

Compensation and Benefits\n
