Key Responsibilities\n

• Evaluate, diagnose, and treat a broad range of psychosocial problems in alignment with the NASW Code of Ethics, ensuring ethical and effective therapeutic practices.

• Collaborate with our Social Work Manager in a multidisciplinary approach, providing valuable consultations regarding patients’ backgrounds and needs to enhance comprehensive care planning.

• Conduct bio-psychosocial evaluations to identify barriers to treatment and discharge, with the ultimate goal of enhancing patient health and resource utilization.
\n
• Develop tailored treatment plans that utilize both individual and group therapeutic modalities, continuously assessing the course and results to optimize each patient’s health status.

• Engage in thorough documentation of all social work activities in patients’ medical records according to departmental standards, ensuring accuracy and compliance.
\n
• Educate patients and their families about medical and social work recommendations, fostering understanding and acceptance to facilitate optimal coping and functioning.

• Facilitate advocacy activities within the community to improve social support systems and resources for our patients, championing self-determination and empowerment.

• Stay informed about available community resources, integrating these into the treatment process for enhanced patient care.

• Participate in ongoing educational activities to maintain and enhance your professional expertise.

• Adhere to departmental policies that promote a safe care environment, ensuring legal and regulatory compliance at all times.

Patient Population / Client Focus\n

This position primarily serves patients dealing with end-of-life issues and those experiencing complex psychosocial challenges related to serious illnesses. You will provide emotional support and counseling to individuals and families, meeting their specific needs with sensitivity and understanding.

Qualifications\n

• A Master’s Degree in Social Work from an accredited graduate program is required, along with a current LCSW license from the State Board of Social Work Licensure.

• The ideal candidate will possess demonstrable knowledge necessary for delivering effective patient care, particularly in consideration of aging processes and cultural competence.

Preferred Experience\n

• Between six months and one year of clinical social work experience in health or mental health settings, particularly with populations similar to those served by our team.

• Experience in hospice or palliative care is highly desirable, as is familiarity with issues related to grief and loss.

Compensation and Benefits\n

Our compensation package includes a competitive salary range of USD 85,000 – 100,000 annually, dependent on experience and qualifications.
