Work Environment

This position is primarily onsite and focuses on home health care, allowing you to provide essential therapy services in the comfort of patients' homes. Key Responsibilities

• Conduct thorough evaluations and assessments of patients with functional limitations and disabilities, developing individualized treatment plans to meet their unique needs.
• Communicate effectively with patients, family members, and healthcare providers, providing consultations regarding the continuum of care.
• Deliver hands-on treatment, including therapeutic exercises, manual therapy, and other interventions, while continuously evaluating patient progress.
• Maintain accurate documentation of treatment plans and patient interactions, ensuring compliance with applicable laws and professional standards.
• Collaborate with an interdisciplinary team, engaging physicians and other stakeholders to provide holistic care.
• Engage in community outreach, program development, and quality improvement initiatives, actively participating in staff meetings and continuing education opportunities.
• Supervise and mentor support staff, ensuring optimal use of departmental resources.

Patient Population / Client Focus

The Home Health Physical Therapist will work with a diverse patient population, primarily consisting of individuals recovering from orthopedic surgeries, neurological disorders, or chronic conditions that impact mobility. Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited program is required.
• A current state license in Physical Therapy is mandatory.

• Competence in electronic medical records (EMR) systems and basic computer skills are necessary for documentation and reporting.
• Basic Life Support (BLS) certification is required.
