Work Environment

This Physical Therapist position is primarily onsite, providing in-home care services to clients. You will be working within the comfort of patients' homes, tailoring therapy interventions in environments that promote healing and recovery. Adopting a concierge-style, on-demand approach ensures our patients receive the highest standard of care in familiar and supportive surroundings.

Key Responsibilities

• Direct and implement individualized physical therapy treatment plans focused on preventing disabilities, relieving pain, enhancing muscle function, and maintaining maximum performance as per each patient's capabilities.
• Supervise and guide physical therapy assistants to ensure adherence to the established plan of care, fostering collaboration and effective communication.
• Conduct comprehensive evaluations of clients' functional capabilities using diagnostic tests that assess muscle, nerve, and joint function.
• Assess home environments and recommend modifications to improve client independence and rehabilitation potential.
• Deliver physical therapy services in the client's home as prescribed by their physician, including manual therapeutic exercises and education on non-manual therapy techniques.
• Administer traction, massage, and various physical agents to facilitate healing.
• Fit, adjust, and evaluate prosthetic and orthotic devices, providing necessary recommendations to physicians.
• Participate actively in interdisciplinary team meetings to devise and review comprehensive care plans for clients.
• Maintain detailed clinical records, including evaluations, progress notes, and discharge summaries, ensuring accuracy for compliance and quality assurance.
• Communicate effectively with clients and their families, educating them about therapy devices and comprehensive therapy programs.
• Assist and instruct Physical Therapy Assistants in therapy delivery where applicable, ensuring compliance with state regulations.
• Perform timely assessments and modifications of care plans as necessary, contributing to ongoing patient welfare and rehabilitation success.
• Complete additional tasks and responsibilities as directed to support the organization's mission and achieve exceptional patient outcomes.

Patient Population / Client Focus

This role focuses on an adult patient population requiring physical rehabilitation interventions within their homes. You will work with diverse clients, ranging from those recovering from surgery to individuals managing chronic conditions, aiming to restore their mobility and enhance their quality of life through tailored therapy solutions.

Qualifications

• Possess a Doctorate in Physical Therapy (DPT) from an accredited institution.
• Current state licensure in good standing and a valid CPR certification.
• A minimum of one year of professional experience in the field of physical therapy, particularly in acute care or home care settings.
• Demonstrated ability to make independent clinical decisions and embrace critical thinking in therapy practice.
• Strong interpersonal skills, with a focus on effective communication and fostering relationships with patients and families.
• A commitment to continuous learning and staying updated on advanced therapies and techniques in physical rehabilitation.

Preferred Experience

• Experience in a home health setting, demonstrating a broad understanding of rehabilitation services in unique patient environments.
• Familiarity with electronic medical records and documentation practices pertinent to home healthcare.
• Proven ability to work as an integral part of an interdisciplinary team.

Compensation and Benefits

This position offers a competitive salary ranging from USD 100,000 to 120,000 annually, based on experience and qualifications.
