Nealy Fitness Rehab Center is seeking a dedicated and compassionate Full-Time Orthopedic Physical Therapist to join our home healthcare team in Freehold, New Jersey. Work Environment

This full-time position is primarily conducted onsite in our clients' homes within Freehold, New Jersey, providing a unique opportunity to deliver specialized care in a familiar and comfortable environment. You will collaborate closely with an interdisciplinary team of healthcare professionals and caregivers, ensuring a holistic approach to treatment and client support.

Key Responsibilities

• Develop individualized treatment plans based on each client's specific needs, aimed at preventing disability, relieving pain, and fostering muscle function restoration.
• Oversee the implementation of care plans by physical therapy assistants, ensuring they follow prescribed protocols and adjusting methods as necessary to optimize client outcomes.
• Conduct evaluations of clients' functional abilities through comprehensive testing of muscle strength, joint mobility, and overall physical condition.
• Assess the home environment and make recommendations for modifications that facilitate greater independence for clients receiving care.
• Provide hands-on therapy that may include manual exercises, traction, massage, and the application of therapeutic modalities.
• Fit, evaluate, and recommend orthotic and prosthetic devices tailored to each client's needs.
• Document patient progress and changes in condition accurately in clinical records, including OASIS assessments and care plans.
• Engage with clients and their families to educate them on therapy techniques and ensure a thorough understanding of the treatment plan and its goals.
• Participate actively in interdisciplinary team meetings to collaborate on total treatment strategies for each client.
• Adhere to all clinical documentation standards and submit accurate reports and orders while complying with regulations and branch policies.

Patient Population / Client Focus

Your primary focus will be on clients in the home care setting who are recovering from orthopedic surgeries, managing chronic illnesses, and those requiring rehabilitation for injuries. Qualifications

• A Bachelor's degree in Physical Therapy from an accredited program; a Doctorate in Physical Therapy is preferred.
• Active licensure as a Physical Therapist in the state of New Jersey.
• Current CPR certification and a valid driver's license with automobile insurance.
• Strong commitment to patient-centered care with the ability to engage effectively with clients and families.
• Excellent interpersonal and communication skills, proficient in collaborating with a diverse range of healthcare professionals.

Preferred Experience

• Minimum of one year of experience in an acute care or home care setting.
• Familiarity with the principles of patient-centered rehabilitation and the ability to adapt treatment plans based on clinical assessments.
• A strong passion for clinical excellence and a motivation for continuous professional development.

Compensation and Benefits

We offer a competitive salary range of USD 100,000 to 120,000 annually, commensurate with experience, along with a comprehensive benefits package including health, dental, and vision insurance, paid time off, and retirement plans to support your well-being both personally and professionally.
